These Terms of Use (“Terms”) govern your access to and use of CareContinuum and related services (the “Services”). By creating an account or using the Services, you agree to these Terms and our Privacy Policy.
If you do not agree, do not use the Services.
The Services are for informational and self-management support only. They are not a substitute for professional medical advice, diagnosis, or treatment. Always seek the advice of a qualified health provider with any questions about a medical condition. Never disregard professional advice or delay seeking it because of something you read in the app.
If you think you may have a medical emergency, call your local emergency number or seek immediate in-person care.
You must provide accurate account information and keep your credentials secure. You are responsible for activity under your account. Notify us promptly if you suspect unauthorized access.
You may permanently delete your account in the app under Settings (Delete account). You must confirm with your current password. Deletion is intended to remove your account and associated app data we hold under your user ID; see the Privacy Policy for details and exceptions.
Subject to these Terms, we grant you a personal, non-exclusive, non-transferable, revocable license to use the Services for your own non-commercial use, unless we agree otherwise in writing.
You agree not to misuse the Services, including: attempting to gain unauthorized access; interfering with security or performance; using the Services to violate law; scraping or reverse engineering except as permitted by law; or uploading malware or harmful content.
Outputs generated by automated or AI features may be inaccurate or incomplete. You should verify important information with a qualified professional. We are not liable for decisions you make based on AI outputs.
The Services may integrate with third-party platforms (for example, cloud infrastructure or AI providers). Their use is subject to their terms and policies. We do not control third-party services.
THE SERVICES ARE PROVIDED “AS IS” AND “AS AVAILABLE” WITHOUT WARRANTIES OF ANY KIND, WHETHER EXPRESS OR IMPLIED, INCLUDING MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT, TO THE MAXIMUM EXTENT PERMITTED BY LAW.
TO THE MAXIMUM EXTENT PERMITTED BY LAW, WE AND OUR AFFILIATES, LICENSORS, AND SUPPLIERS WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, OR ANY LOSS OF PROFITS, DATA, OR GOODWILL, ARISING FROM YOUR USE OF THE SERVICES.
OUR TOTAL LIABILITY FOR ANY CLAIM ARISING OUT OF THESE TERMS OR THE SERVICES WILL NOT EXCEED THE GREATER OF (A) THE AMOUNTS YOU PAID US FOR THE SERVICES IN THE TWELVE (12) MONTHS BEFORE THE CLAIM OR (B) ONE HUNDRED U.S. DOLLARS (US$100), IF APPLICABLE LAW ALLOWS SUCH A CAP.
To the extent permitted by law, you agree to indemnify and hold harmless us and our affiliates from claims, damages, losses, and expenses (including reasonable attorneys’ fees) arising from your use of the Services or violation of these Terms.
We may suspend or terminate access to the Services if you violate these Terms or if we need to do so for legal, security, or operational reasons. You may stop using the Services at any time.
You may self-serve permanent account deletion in the app (Settings → Delete account) as described in the Privacy Policy.
These Terms are governed by the laws applicable to your organization’s jurisdiction, excluding conflict-of-law rules. Courts in that jurisdiction may have exclusive jurisdiction over disputes, unless mandatory consumer protections in your country require otherwise.
We may modify these Terms. We will provide notice as required by law or through the app. Continued use after changes may constitute acceptance.
